The GT NOMINATION window is officially open this week, and will remain open until JANUARY 16th! Below are all the forms you’ll need for nominating students, and for parents who inquire. I will send out communication through ParentSquare but oftentimes, parents will inquire about information from you. I have also attached the flyer that will be going out to Families. Here’s everything you’ll need!
- The first two forms are for PARENTS ONLY (one in English and the other is in Spanish). These are what you’ll send parents who’d like to nominate a student. We’ll need a teacher and a parent form for every student nominated, regardless of whom they’re nominated by.

- The third form is your TEACHER REFERRAL FORM. This is what you’ll use to nominate a student for GT Testing. If you nominate a student, remember to also inform their parents and forward them the Parent Input Form, as again we’ll need both.
- Teacher Referral Form
- The final form is the GT REFERRAL LIST. This one’s quick and simple: on this form you’ll just list each of your students who’s been nominated, either by you, or by a parent.

We’ve tried to minimize as much inconvenience as we can by eliminating physical paperwork, but we realize it still requires time on your part, and we deeply appreciate everything you do to help us identify students who require services.
